Application for Registration or Recordation

Your ADGA ID Number (This form not available for use by nonmembers.  For information on joining ADGA, please go to www.adga.org)

1. GOAT'S NAME: (Name must be confined to 30 letters or spaces, and must begin with the breeder's herd name, if any.  Name cannot be changed once registered.)

If  your choices of names are not available, should ADGA:

Artificial Insemination Mating?   Yes   No  (If yes, you must attach the A.I. memo to your application.

2. NAME OF SIRENUM.
If the official ownership of the sire and that of the dam are not identical on the date of breeding, a service memo must be attached.

3. NAME OF DAMNUM.
If person making application for registry purchased the dam as a bred doe, a service memo must be obtained from the owner of the sire and attached to the application.   Dam must be officially transferred before kid may be registered.

5.  COLOR AND MARKINGS:   Limited to 69 letters and spaces. 
Be accurate but make description as short as possible.  For Toggenburgs indicate shade of brown and color of correct markings.  For Oberhasli indicate shade of bay.

After you have completed the necessary fields of information, you are ready to  preview the results, by using the "Show Me!" button. (YOU MUST PREVIEW (BY PRESSING THE "SHOW ME" BUTTON) IN ORDER TO PRINT YOUR COMPLETED FORM!)   Proofread carefully, and if you see things to correct, simply use your back arrow and make the changes.  When you have made any necessary changes, just preview for the final time, print the form and mail or fax to ADGA with payment information.  If you have other littermates to register, just enter the new information when you return to this screen, and you won't have to put it all in again! (Just make sure you correct the tattoo fields and color descriptions!)  If the form does not print on one single page, then reduce the font size in your browser.

NOTE:  If you have used this form to create a registration, we would like to hear from you, so we will have some idea of how many  registrations are actually being created and used.  It's not required, but will help give us important feedback!
